Are Massage Services Offered by Physios GST Free or Taxable?

GST classification is not just a formality. It directly affects your pricing, invoicing and reporting obligations. 


A common misconception is that all massage services provided by physios are GST free, but that is not always the case. Whether you’re a solo practitioner or run a multidisciplinary clinic, knowing how to distinguish between GST free and taxable services helps you: 


  • Stay compliant with ATO regulations 

  • Maximise legitimate deductions 

  • Avoid penalties or back payments 

  • Build accurate and professional pricing structures 


If your practice includes both clinical treatments and wellness services, understanding the difference can be a little tricky. That is why it pays to be informed. 


When Are Massage Services GST Free? 

According to the ATO, massage services provided by a registered physiotherapist are GST free only when: 

  • The massage is part of a treatment for a specific medical condition 

  • It is provided during a consultation aimed at preventing or treating a health issue 

  • The practitioner is registered with AHPRA (Australian Health Practitioner Regulation Agency) 


Example: 

A patient receives massage as part of a rehabilitation plan after surgery. The service is considered GST free because it's integrated into a clinical treatment plan. 


When Are They Taxable? 

Massage services are not GST free (i.e. GST applies) if: 

  • They are offered as general relaxation or wellness treatments 

  • There is no link to a clinical diagnosis or treatment plan 

  • The massage is booked independently and not in the context of physiotherapy care 

 

Example: 

A client books a 60 minute relaxation massage at your clinic, unrelated to any injury or rehab. This would typically be subject to GST. 


Best Practices for Physiotherapists 

To manage GST properly, consider these tips: 

  • Document everything and maintain clear treatment notes when massage is used as part of physiotherapy 

  • Separate taxable vs GST free services, especially if you offer both types at your clinic 

  • Label invoices properly to indicate whether GST was applied and why 

  • Train admin staff to ensure they understand the difference when processing bookings and payments 

  • Use accounting software and make use of tools that can split GST free and taxable items for BAS reporting 


Common Mistakes to Avoid 

  • Assuming all services by a physio are automatically GST free 

  • Bundling relaxation massages under physiotherapy invoices 

  • Failing to register for GST when required (if turnover exceeds $75,000) 

  • Not clearly communicating GST charges to clients
